Position Title: Nurse Practitioner – Diabetes Specialty

Location: Shkagamik-Kwe Health Centre (Sudbury, Ontario)
Employment Type: Full-Time, Permanent (35 hours/week)
POSITION SUMMARY:
At Shkagamik-Kwe Health Centre (SKHC), we walk in balance guided by the Seven Grandfather Teachings, the wisdom of our Elders, and Anishinabek traditions. The Nurse Practitioner (NP) – Diabetes Specialty provides wholistic and culturally safe care to clients living with, or at risk of, diabetes. This includes advanced clinical assessment, diagnosis, treatment, health promotion, and chronic disease management.
The NP integrates Western medicine with Indigenous knowledge, ceremony, and teachings to support healing of the physical, mental, emotional, and spiritual aspects of wellness. Through collaboration with physicians, dietitians, Traditional Healers, and interdisciplinary team members, the NP will strengthen pathways of diabetes prevention, management, and wellness for individuals, families, and communities.
KEY RESPONSIBILITIES:
- Provide advanced diabetes care (Type 1, Type 2, gestational), including assessment, diagnosis, treatment, and ongoing management.
- Prescribe and manage medications and insulin therapies in alignment with NP scope of practice and CNO standards.
- Order and interpret diagnostic tests (A1C, renal function, lipid profile, etc.) to inform care planning.
- Create individualized care plans with clients and families that honour personal goals and Anishinabek ways of knowing.
- Address co-morbid conditions such as hypertension, obesity, mental wellness, and cardiovascular risk factors.
- Provide culturally safe education on diabetes prevention and management to clients, families, and community members.
- Collaborate with Traditional Healers, Knowledge Keepers, and Elders to integrate ceremony, seasonal teachings, and cultural supports into care.
- Support community-based diabetes screening, prevention, and early intervention initiatives.
- Promote healthy living through both Western approaches (nutrition, physical activity, counselling) and land-based, traditional practices.
- Work as a key member of the interdisciplinary Primary Care and Diabetes teams.
- Collaborate with internal staff and external partners, including hospitals, specialists, and Indigenous organizations, to ensure continuity of care.
- Actively participate in SKHC cultural events, gatherings, and wellness programming as part of integrated client care.
- Contribute to diabetes program planning, quality improvement, and evaluation.
- Maintain accurate and timely documentation within the EMR system.
- Advocate for equitable, culturally safe diabetes care for Indigenous people.
- Support SKHC’s strategic directions by weaving together medical best practices and Indigenous healing pathways.
QUALIFICATIONS:
- Current registration as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario in good standing.
- Master’s Degree in Nursing or equivalent.
- Minimum 2 years’ clinical experience in primary care or chronic disease management; direct diabetes care experience preferred.
- Certified Diabetes Educator (CDE) designation is a strong asset.
- Knowledge of Anishinabek teachings, cultural safety, and wholistic care approaches (or willingness to learn with guidance from Elders).
- Experience working with Indigenous communities considered an asset.
- Strong interpersonal, communication, and critical thinking skills.
- Commitment to reconciliation, equity, and Indigenous self-determination in health care.
WORKING CONDITIONS:
- Based in Sudbury at SKHC, with some outreach to surrounding First Nation and urban Indigenous communities.
- Flexibility to work occasional evenings and weekends to meet community needs.
- Participation in ceremony, cultural events, and community gatherings will be supported as part of the role.
